ESSENTIAL FUNCTIONS: The Behavioral Health Clinician will be part of a robust Community Health Team at FSRI. The Behavioral Health Clinician will provide treatment of referred patients and assist with barriers to care. Staff will conduct assessment screenings; treatment planning; evidence based therapeutic intervention; care coordination; consultation; and crisis evaluations on site at a medical practice, patient’s homes, in office and other community venues as appropriate. The Behavioral Health Clinician will also provide supervision of assigned Community Health Workers, clinical support, and oversight of cases involved with FSRI’s CHT as well as participate on the agencies after on call system where indicated.


Conduct a clinical assessment utilizing a standardized tool for assigned patients and their families.
Complete an individual/family action plan that addresses identified needs.
Provide clinical interventions to patients utilizing evidence informed practices.
Link and refer individuals/families to appropriate culturally relevant resources and informal supports within their community when that is appropriate and sufficient to meet their need(s).
Provide crisis intervention evaluation/ or services with and on behalf of, patient/family as need.
Provide clinical consultation to medical staff and colleagues.
Provides administrative and clinical supervision to assigned Community Health Workers; and oversight of colleagues who share cases within the program.
Advocate for the peer specialist and peer specialist services across the organization and in the community.
Complete all required documentation via agency EMR and other sources.
Complete data collection and data entry as required.
Complete training required to be a certified and/or licensed within position expectations.
Participate in internal and external meetings as needed.
Act as an ambassador of FSRI through the use of social media and other forms of communication; sharing employment opportunities, FSRI activity and initiatives.
Participate in ongoing supervision.
Other duties as assigned.


Master’s degree required; LCDP preferred; Independent license eligible required (LICSW, LMHC, LMFT)
Certification for supervising certified peer recovery specialist(s)
Valid driver’s license and reliable transportation.
Ability to use an Electronic Medical Record, all MS Office programs, mainframe computer systems and Internet.
Ability to provide services in patient’s homes, medical clinics and community locations and at other agency sites.
Ability to have a flexible schedule to include some evenings.
Prior experience in behavioral health and/or crisis intervention preferred.
Spanish bilingual preferred.
Compliance with state requirements for health clinic employees.


This position requires agency and community visits, employees in this position must have the ability to:

Obtain all necessary immunizations since the position requires working in healthcare settings.
Follow professional dress codes of both FSRI and primary care sites inclusive of not having casual dress down days.
Travel to and from community locations and agency sites, which could include using walkways, stairs and/or elevators.
Ability to lift up to 20lbs.
Ability to communicate effectively.

Receive a Sign-on Bonus in the amount of $2,000.00 within 1 year of employment!
Bilingual Clinicians receive a Sign-On Bonus in the amount of $2,500.00 within 1 year of employment!
