ESSENTIAL FUNCTION: The CHW will work as part of a multidisciplinary team in partnership with health care and other service providers to assess and reduce barriers to care for medically and/or behaviorally complex patients and families. Primary responsibilities include completing individual and family need assessments, developing action plans, care coordination, ensuring referrals made are appropriately matched to needs, and providing ongoing patient support and monitoring. The CHW will also conduct Screening Brief Intervention and Referral Treatment (SBIRT) screenings. Services are provided virtually, in patient’s homes, health care sites, community locations and at other agency sites.



SPECIFIC RESPONSIBILITIES:

Conduct a full needs assessment utilizing standardized tools for assigned patients.
Complete an individual action plan that addresses current identified needs.
Link and refer individuals to appropriate culturally relevant resources and informal supports within their community.
Provide coordinated care and monitoring through contact with individuals, medical entities, community providers and referral sources.
Utilize specific outcome measurement screening tools; including SBIRT, GPRA and others in accordance with the protocol outlined at the SBIRT training.
Teach self-advocacy skills.
Complete all required documentation via agency EMR and other required sources.
Complete data collection and data entry as required.
Complete all professional and administrative documentation and forms in accordance with agency standards and within established guidelines and timeframes.
Maintain or exceed productivity expectations.
Build and maintain positive working relationships with medical entities, community organizations and service providers.
Complete training required for position including certification within six month or maintaining status as a Community Health Worker.
Ability to work virtually and use telehealth.
Participate in ongoing supervision and staff meetings.
Other duties as assigned.

QUALIFICATIONS:

Associates or Bachelor’s degree preferred, High School Diploma or Equivalent required.
Community Health Worker Certification preferred.
Ability to meet all requirements to be certified as a Community Health Worker within the first 6 months of employment.
Bilingual in Spanish or Portuguese preferred.
Valid driver’s license and reliable transportation.
Ability to work independently and as an interdependent team member.
Ability to provide services virtually, in patient’s homes, primary health care sites, community locations and at other agency sites.
Ability to have a flexible schedule to include some evenings.
Prior experience in behavioral health and/or crisis intervention preferred.


PHYSICAL REQUIREMENTS:

This position requires agency and community visits, employees in this position must have the ability to:

Travel to and from community locations and agency sites, which could include using walkways, stairs and/or elevators.
Obtain all necessary immunizations since the position requires working in healthcare settings.
Follow professional dress codes of both FSRI and primary health care sites.
Ability to use Personal Protective Equipment as needed.
Ability to lift up to 20lbs.
Ability to communicate effectively.
