Conduct community outreach within Central Falls to identify residents in need of program supports; conduct client screenings and regular follow ups via phone and in person to provide COVID-19 related counseling support, education, screening/assessment of need, and linkage to other resources. Connect recipients with behavioral health treatment, basic needs support, and physical health resources to help them effectively cope with crises related to COVID 19 and its many impacts. Responsible to deliver Be Safe kits to Central Falls households who are in need.


Specific Responsibilities

Work in partnership with the Housing Support Specialist and Program Supervisor to conduct community outreach for the purpose of identifying residents and households in need of program services.
Outreach and respond to an assigned caseload of people in crisis via phone or in person while working remotely, in the office, and/or in community settings.
Deliver Be Safe kits to Central Falls residents in need; utilizing Be Safe contact-free delivery protocols.
Determine needs of those in crisis and provide education, supportive counseling, and linkages to other resources as needed including internal and external programs and services.
Complete all required documentation and data entry for the CDBG funded Be Safe Housing Support Program in a timely manner to ensure correct service information is captured and tracked accurately.
Collaborate with other members of the program team to ensure program goals are achieved and appropriate responses for those served as well as timely follow up when needed.
Maintain contact with individuals, families and referral partners to assure requested assistance is obtained and referrals are completed.
Act as community liaison for program and the agency as a whole when needed.
Provide language support in collaboration with internal programs as needed.
Link and refer client/families to appropriate culturally relevant community based resources and informal supports within their community when that is appropriate and sufficient to meet the families need(s)
Provide on-going phone and/or in person support to connect individuals and families with appropriate services
Participate in ongoing supervision and staff meetings.
Other duties as assigned


AA degree in human service related field required; BA preferred.
Prior experience providing crisis and/or supportive counseling and/or case management required.
Bilingual (English/Spanish) required. Additional language capacity highly preferred.
Ability to provide supportive counseling and targeted assessment of needs relating to the COVID-19 emergency and resulting impacts.
Ability to educate individuals and groups of people regarding the COVID-19 emergency and related issues including crisis responses as well as supports available to assist them with coping with the multiple, on-going impacts from COVID-19.
Ability to use all MS Office programs, mainframe computer systems and Internet.
Ability to work independently and as an interdependent team member.
Ability to meet deadlines and timeframes regarding service provision, follow-up and documentation.
Ability to provide services telephonically and virtually as well as in homes, community locations, and at FSRI sites.


This position requires agency and community visits, employees in this position must have the ability to:

Travel to and from community locations and agency sites, which could include using walkways, stairs and/or elevators.
Ability to lift up to 20lbs.
