ESSENTIAL FUNCTIONS: The Registered Nurse ensures that children and their family receive the services authorized under the First Connections Program.


Performs assessments and provides direct contact, care and comprehensive, community based nursing services for infants and families referred to the First Connections program.
Performs duties and activities with individual children and families in their homes;
Identifies child and family need, educates families, and connects families to community based resources.
Ensures quality service delivery by members of FC team to families referred to program.
Participates in community outreach initiatives and activities as required or requested by clinical supervisor or administrative director.
Participates in ongoing reflective supervision with First Connections Supervisor as scheduled.
Completes all RI DOH and Family Service paperwork including evaluations and written documentation of all communications with families, physicians, referral sources, community partners, etc.
Other duties as assigned or requested by supervisor or director.


Licensed as RN by the RI DOH.
Graduate of Board approved and accredited nursing program.
Minimum 3-year experience in public health, pediatric nursing or community based care.
Minimum of 3-year experience working with young children and their families.
Current and valid drivers license and auto insurance.
Current CPR certification.
As needed to maintain individual licensure and meet RI DOH standards.
Participation in semi-annual and annual employee performance review process including development and review of professional/program goals and objectives.
Other trainings as required by Rhode Island Department of Health.


Travel to and from clients residence, community locations and office site, which could include using walkways, stairs and/or elevators.
Ability to lift up to 20lbs
Ability to communicate effectively.
