ESSENTIAL FUNCTION: As part of the CCBHCs multidisciplinary team, the Nurse Care Manager will assess individuals/families with behavioral health and/or substance abuse issues and provide treatment and referral services. The Nurse Care Manager will lead the development and ongoing management of a culturally competent, comprehensive, individualized care plan with each patient.


SPECIFIC DUTIES/RESPONSIBILITIES:

Provide service delivery and oversight utilizing a multidisciplinary team approach.
Responsible for providing comprehensive screenings, assessment, and care coordination services.
Complete an individual treatment plan that addresses behavioral/medical concerns.
Provide crisis intervention evaluation/ or services with patients as needed.
Coordinate care with other CCBHC staff and any external providers.
Complete all required documentation via EMR and other sources.
Participate in CCBHC internal and external meetings as needed.
Responsible for supervising Case Managers.
Other duties as assigned.


QUALIFICATIONS:

Bachelors degree in nursing (BSN) and a RN licensed in the state of RI.
Experience working in a healthcare setting, preferably in care management.
Experience with children, adolescents and adults from diverse social, cultural and ethnic backgrounds.
Bilingual language skills preferred.
Experience providing care to adults and children with mental health and substance use disorders.
Ability to work independently and part of a team.


PHYSICAL REQUIREMENTS:

Ability to travel to office and community locations (some local travel may be necessary to support CCBHC operations but the vast majority of work will be clinic-based for this position).
Ability to sit for extended periods of time.
Ability to communicate effectively.
Ability to utilize a computer to complete documentation.
Current drivers license, reliable transportation, registration and auto insurance.
