Job Description

Position Summary: The position is co-located at Jenks Park Pediatrics office in Central Falls and a role that promotes equity and inclusion. The CHW will be responsible for actively engaging with families residing in Pawtucket and Central Falls, to identify areas of need by implementing Social Determinants of Health and other screening tools, and ensure referrals are made to the appropriately matched services. CHW will complete individual needs assessments in a variety of settings, including virtually when appropriate, on-site in offices, in resident homes, and in other community venues as appropriate. CHW will also provide individuals with PPE upon request, provide individuals with accurate, up to date COVID-19 prevention materials and connect individuals to community-based vaccination events. CHW is expected to complete home visit to support individuals as needed, and participate in the Pawtucket Central Falls Rhode to Equity project initiatives and attend community meetings when appropriate.


Qualifications:

Associates or Bachelor’s degree preferred, High School Diploma or Equivalent required.
Certified Community Health Worker, or ability to be certified within first 6 months of employment.
Bilingual in Spanish required.
Valid driver’s license and reliable transportation.
Candidates from populations disproportionately impacted by COVID-19 residing in Pawtucket or Central Falls are strongly encouraged to apply.
Ability to document in and navigate an Electronic Medical Record (EMR).
Strong interpersonal and social skills with an ability to collaborate with a variety of individuals.
Ability to provide services virtually, in resident’s homes, and community locations.
Ability to have a flexible schedule to include evening shifts and occasional weekends.
Prior experience in behavioral health and/or case management preferred.
Bilingual skills are compensated!!
