Job Description

Position Summary: The CHW role promotes equity and inclusion. CHW will work as part of a multidisciplinary team supporting various Health Division programs to ensure service deliverables are achieved and support implementation of the Caring Dads program. CHW must be flexible working at Jenks Park Pediatrics office in Central Falls and other community-based location as needed. The CHW will be responsible for actively engaging with residence residing in Pawtucket and Central Falls, to identify areas of need by implementing Social Determinants of Health and other screening tools, and ensure referrals are made to the appropriately matched services. CHW will also provide individuals with PPE upon request, and provide individuals with accurate, up to date COVID-19 prevention materials and connect individuals to community-based vaccination events upon request. CHW is expected to complete home/community visits to support individuals as needed; and lead Pawtucket Central Falls Community Health Worker Networking Meetings; and participate in other programmatic meetings. Additionally, the CHW will serve as the “Mother Contact” connecting with the identified partner to discuss Caring Dads program components, complete a Social Determinants of Health screening, identify needs and ensure referrals are made to the appropriately matched services and provide ongoing support and monitoring.


Qualifications:

Associates or Bachelor’s degree preferred, High School Diploma or Equivalent required.
Certified Community Health Worker, or ability to be certified within first 12 months of employment
Bilingual in Spanish required
Valid driver’s license and reliable transportation
Ability to document in and navigate an Electronic Medical Record (EMR)
Strong interpersonal and social skills with an ability to collaborate with a variety of individuals
Ability to provide services virtually, in resident’s homes, and community locations
Ability to have a flexible schedule to include evening shifts and occasional weekends
Prior experience in behavioral health and/or case management preferred
Bilingual skills are compensated by an additional 6%, above base pay.
