Job Description

Position Summary: The CHW will work with children, adolescents and their families statewide. The CHW will screen families for Social Determinant of Health (SDOH) barriers and connecting them to health and community resources as appropriate. This role is responsible for engaging clients; implementing Social Determinants of Health and other screening tools; identifying areas of need; and ensuring referrals are made to appropriately matched services. CHW will complete individual needs assessments in a variety of settings, including schools, pediatrician offices, in family homes, and in other community venues as appropriate. The CHW will work collaboratively with Best Start team (Clinicians, Nurses, and CHW) and with other FSRI programs and external community-based programs and providers.
Qualifications:
Associates or Bachelor’s degree preferred, High School Diploma or Equivalent required.
Certified Community Health Worker, or ability to be certified within first 6 months of employment.
Experience working with the childhood and adolescent population.
Bilingual in Spanish required.
Valid driver’s license and reliable transportation.
Candidates residing in South Providence are strongly encouraged to apply.
Ability to document in and navigate an Electronic Medical Record (EMR).
Strong interpersonal and social skills with an ability to collaborate with a variety of individuals.
Ability to provide services virtually, in resident’s homes, and community locations.
Ability to have a flexible schedule to include evening shifts and occasional weekends.
Prior experience in behavioral health and/or case management preferred.
Physical Requirements: This position requires agency and community visits, employees in this position must have the ability to:
Travel to and from community locations and agency sites, which could include using walkways, stairs and/or elevators.
Ability and willingness to utilize required Personal Protective Equipment as required.
Follow FSRI professional dress code.
Ability to lift up to 20lbs.
Ability to communicate effectively.
