Essential Functions:
The Community Health Advocate (CHA) will work in unison with the primary care team at Providence Community Health Center (PCHC) to identify and remove barriers in order to close gaps and to facilitate patients in obtaining quality health care. The CHA will provide outreach through telephonic, clinic and community-based visits to high risk adult and pediatric patients of the practice by utilizing assessment tools to reach goals. This position is embedded in PCHC and supports Care Management and Accountable Entity related programs and initiatives.
Qualifications:
Bachelor degree required.
Spanish bilingual required; must pass online course on medical Spanish within 3 months.
Community Health Worker Certification preferred.
Ability to meet all requirements to be certified as a Community Health Worker within the first 6 months of employment.
Prior experience in primary care and behavioral health and/or crisis intervention preferred.
Experience working in the human service field with vulnerable populations including homeless, substance use disorders and severe and persistent mental illness required.
Ability to provide services remotely, in primary health care sites and community locations.
Ability to train colleagues to provide interventions and support to Primary Care sites with a focus on Population Health and Wellness preferred.
Ability to work independently and as an interdependent team member.
Ability to have flexible schedule.
Ability to engage various levels of medical providers in a consultative manner preferred.
Access to reliable internet connection, or ability to navigate organizational hotspot.
Basic knowledge and use of Microsoft Office and other virtual platforms.
Valid driver’s license and reliable transportation.
Ability to communicate effectively.
Follow professional dress codes of both FSRI and primary health care sites.
Physical Requirements: This position requires residential and community visits, employees in this position must have the ability to:
Travel to and from community locations and agency sites, which could include using walkways, stairs and/or elevators.
Obtain all necessary immunizations since the position requires working in healthcare settings.
Ability to use Personal Protective Equipment as needed.
Ability to lift up to 20lbs.
